As a Primary Care Physician (PCP) , you will lead a dedicated multidisciplinary team focused on delivering high-quality, value-based care to seniors. Your role will be pivotal in providing personalized, coordinated, and preventative health services that truly make a difference in our patients' lives.

Your Role in Our Team

As a PCP, you will thrive in a collaborative, team-based environment. Our care team includes:

  • Care Coach Nurses

  • Quality-Based Coders
  • Referral Coordinators

This robust support allows you to focus on fewer patients, ensuring you can spend quality time with each one, enhancing their care experience.

Key Responsibilities

  • Evaluate and treat patients in accordance with established standards of care.
  • Monitor patient care quality using available data and chart reviews.
  • Coordinate patient services, including specialty referrals and home health care.
  • Actively participate in daily huddles, providing medical expertise to the care team.
  • Maintain timely medical record documentation with the help of quality-based coders.
  • Engage in discussions about quality of care and outcome data with leadership.
  • Explore growth opportunities for new or existing services within the center.
  • Participate in the local primary care “on-call” program as needed.
  • Ensure compliance with all licensing and regulatory standards.
  • Dedicate 100% of your time to direct patient care and related administrative tasks.

What We’re Looking For

Required Qualifications:

  • Current and unrestricted medical license or willingness to obtain one in the state of practice.
  • Graduate of an accredited MD or DO program.
  • Excellent verbal and written communication skills.
  • Strong interpersonal skills with patients and colleagues.
  • Commitment to integrated team-based care.
  • Willingness to adapt to a value-based care setting.
  • Basic computer skills, including email and EMR.
  • Participation in TB screening program required.

Preferred Qualifications:

  • Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active and unrestricted DEA license.
  • Experience managing a Medicare Advantage panel of patients.
  • Knowledge of Medicare guidelines and HEDIS quality indicators.
  • Bilingual skills are a plus.
